  • The person in charge failed to demonstrate managerial control of the facility.
  • An employee was observed washing hands in the meat preparation sink situated near the back door.
  • A pan of red pepper sauce was found with a white mold-like substance present.
  • Raw beef and raw seafood were seen stored behind ready-to-eat foods in open-top prep coolers.
  • The report cited improper food storage, including raw beef and pork stored above ready-to-eat items and unwashed vegetables stored above washed vegetables.
  • Several containers of raw and ready-to-eat foods were found uncovered in prep coolers, walk-in coolers, and upright refrigeration units.

  • The report found that the walk-in freezer had ice accumulating on the floor, and its door was not closing properly.
  • The facility was using quaternary ammonium sanitizer in buckets and spray bottles. It did not have test strips to verify sanitizer concentration.
  • Live bugs were observed near the front handwashing sink and inside an open-top prep cooler on a dish of uncovered food.
