AIKEN, S.C. (WRDW/WAGT) – The Aiken County Sheriff’s Office and Rural Health Services have launched a partnership aimed at connecting people leaving the Aiken County Detention Center with primary health care soon after release.
The program, called Project Connect, is designed to improve continuity of care for justice-involved individuals re-entering the community, with a focus on people managing chronic conditions such as high blood pressure and diabetes, officials said.
Under a two-year memorandum of understanding effective April 15, Rural Health Services will provide transitional primary care services for eligible people being released, including registration and intake assessments, insurance enrollment support, care coordination and connections to ongoing medical and behavioral health services, according to the sheriff’s office…
