GARDENDALE, Ala. — The Centers for Medicare and Medicaid Services (CMS) announced it has terminated its Medicare provider agreement with Magnolia Ridge. This decision follows the facility’s failure to comply with Medicare and Medicaid health and safety participation requirements.
The Alabama Department of Public Health said officials conducted three recent surveys of Magnolia Ridge, revealing several deficiencies, including those involving immediate jeopardy. As a result, the facility remains out of compliance with the Centers for Medicare and Medicaid Services’ Conditions of Participation.
The CMS notice of involuntary termination, issued on September 4, 2025, states that Magnolia Ridge, located at 420 Dean Drive in Gardendale, will no longer receive Medicare payments for services provided to beneficiaries admitted on or after April 22, 2025.
