VA inspector general report finds ‘deficiencies in care’ contributed to patient’s death in Western N.Y.

The VA inspector general has released a new report that says “deficiencies in care” may have contributed to a patient’s death in Western New York.

The report says that last year, a patient with a history of dementia, anxiety and diabetes was admitted to the Buffalo VA.
