Former Wellbrooke of Carmel employee accused of controlled substance fraud

A former employee at the Wellbrooke of Carmel assisted living facility is accused of diverting oxycodone tablets over several months in 2025.

Tamara Smart, 51, is facing two felony charges: obtaining a controlled substance by fraud and failure to make, keep or furnish a record. The Indianapolis resident was arrested June 17 and released from the Hamilton County Jail the same day after posting a $15,000 bond.
