CEDAR RAPIDS, Iowa — An Iowa care facility in Cedar Rapids has been fined by state regulators after inspectors found resident gift cards had been spent fraudulently and improperly stored. The violations were uncovered during a September inspection by the Iowa Department of Inspections, Appeals and Licensing.
According to state inspection records, a Walmart gift card belonging to a resident at REM Iowa’s care facility on 33rd Avenue in Cedar Rapids was found in September to have been spent fraudulently, with no receipts available to account for the purchases.
