Norfolk shipyard fined $16,550 after fatal accident in October 2024

NORFOLK, Va. — Marine Hydraulics International (MHI) has been fined $16,550 by OSHA following a fatal industrial accident on October 23, 2024, at its Norfolk facility.

The accident occurred aboard the USNS 2nd Lt. John P. Bobo when rigging gear was improperly attached to pre-installed U-bolts that failed to support an elevator. The elevator fell, striking two subcontractor employees. Both men were critically injured; one was pronounced dead at the scene, and the other died later at the hospital.

MHI confirmed both workers were independent contractors and expressed deep sorrow over the tragedy. “Our thoughts and prayers are with their families, as well as with the many impacted workers at our facility. Safety is our top priority, and we are cooperating with authorities to investigate the cause of this incident,” the company said…
