3 Miami Valley Medicaid providers charged with fraud; over $530k allegedly stolen

DAYTON, Ohio (WDTN) – Three Miami Valley Medicaid providers are facing fraud charges for allegedly stealing funds from the government healthcare program.

According to indictments filed by Ohio Attorney General Dave Yost’s office, nine Medicaid providers across Ohio have been accused of stealing over $530k in Medicaid funds, per the indictments filed in November 2025.

Among those nine are Mustafa Issa, 34, and his wife Ayshia Mustapha, 28. The two are accused of running multiple billing schemes out of their West Chester business, Hearts of Care Home Health Care Agency.

Police warn against leaving cars running unattended

According to the Medicaid Fraud Control Unit, they caused a loss of over $344k to Medicaid between June 2023 and November 2024.
