The Iowa Board of Nursing has suspended the license of an Ankeny nurse following a criminal conviction tied to identity theft in Polk County, Iowa.
Jamie Jorgensen, 38, of Ankeny, previously faced two counts of forgery stemming from allegations that a physician’s signature was forged on medical records. Police records indicate the alleged incident occurred at 21st Century Rehab in Altoona, Polk County.
