County Completes Updated Community Health Assessment, Improvement Plan

The Chautauqua County Health Department (CCHD), in collaboration with the Brooks-TLC Hospital System, UPMC Chautauqua, and Westfield Memorial Hospital, recently announced the completion of the 2025-2030 Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). County health officials say the CHA and CHIP align with the New York State Prevention Agenda, the state’s health improvement plan that serves as a comprehensive framework for local and statewide action to improve health outcomes and reduce health disparities. Developed and updated by the New York State Public Health and Health Planning Council, the Prevention Agenda emphasizes prevention and equity, with the vision that every New Yorker has the opportunity to achieve their highest level of health across the lifespan.
