Oregon Youth Authority investigator failed to review thousands of cases, report finds

The former top investigator at the Oregon Youth Authority failed to review and finalize more than 3,000 investigations into abuse complaints and almost a third of all cases opened since 2018 remain incomplete, according to records the agency released Thursday.
