New program brings healthcare support into patients’ homes

Lee Health is constantly evolving and introducing innovative ways to care for our patients and neighbors throughout our community—telehealth, e-visits, Hospital at Home and in-home urgent care through DispatchHealth are a few examples. We are now partnering with Lee County Public Safety to launch a Community Paramedicine program to help patients who may need extra care and support between their discharge from the hospital and follow-up physician office visits. We are introducing the program first at Cape Coral Hospital.

What is the Community Paramedicine Program?

Community Paramedicine is a service designed to help patients recover safely at home while staying connected to the care they need. Once referred to the program, and within 24-48 hours, a specially trained Community Paramedicine team member will visit the patient’s home to ensure they are safe, on track and supported in their recovery. The Community Paramedicine team can include emergency medical technicians (EMTs), paramedics, case managers and social workers.
