- On October 2nd 2025 at about 1:55 AM, CHP, San Bernardino County Fire, and AMR responded to a reported traffic collision on the Southbound 215 Freeway, just South of Orange Show in San Bernardino.
- CHP arrived at the scene and located a semi with dual trailers overturned and the cab on fire.
- San Bernardino County Fire arrived, extinguished the fire, and unfortunately located the driver deceased inside.
- CHP has requested CalTrans to respond to the scene for a hard closure while officer investigate the fatal collision.
