Woman’s death at H-E-B warehouse prompts federal and local investigations

SAN ANTONIO – Federal authorities are looking into why a woman working at an H-E-B warehouse died on the job last month. We have confirmed the Occupational Safety and Health Administration (OSHA) has launched an investigation into the death of 27-year-old Teresa Dominguez.

Dominguez died on October 24 at the warehouse along the 5700 block of Rittiman Road on the Northeast Side. She was found unconscious inside a freezer and was later pronounced dead at the hospital.

Initial police reports indicate she died from a forklift accident. H-E-B disputed that report and said her death happened because of what they called “a medical episode.”…
