Columbus fire chief to face disciplinary charges after investigation into fatal crash

COLUMBUS, Ohio (WCMH) – An independent investigation into a fatal crash involving a Columbus Division of Fire firefighter will result in disciplinary charges filed against the division’s chief.

The investigation found multiple violations within the department pertaining to the investigation of the 2023 crash and the lack of discipline for the firefighter involved, with the Columbus Director of Public Safety filing disciplinary charges against Columbus Division of Fire Chief Jeffrey Happ.

The report also determined firefighter Tyler Conners was at fault for the 2023 traffic crash that killed one man and cost the city millions of dollars.

According to the report, Conners was the driver of the engine on the night of July 23 when the crash happened on Interstate 270 near the Hamilton Road exit. The engine and its crew were in “non-emergency travel” when the crash happened.
