‘Completely preventable mistake’ | Federal report out on deadly chemical release at Deer Park refinery

DEER PARK, Texas — Federal investigators released their report Monday into the fatal Oct. 10, 2024, release of a toxic chemical at the PEMEX Deer Park Refinery in Deer Park.

Two contract workers died from exposure to hydrogen sulfide gas, while 13 others were transported to local medical facilities. Dozens more were treated at the scene. Shelter-in-place orders were issued for two neighboring cities.
