Coroner identifies man who died from medical neglect

CHARLESTON COUNTY, S.C. (WCBD) – The Charleston County Coroner’s Office on Wednesday publicly identified a 64-year-old who died from medical neglect while living in a North Charleston residential care facility.

Coroner Bobbi Jo O’Neal said Thaddeus Moose, of Charleston, was taken to the Ralph H. Johnson VA Medical Center on April 10, 2025, and died four days later on April 14.

An extensive investigation determined the cause of death to be septic shock, bronchopneumonia, and dehydration caused by multiple stage 4 decubitus ulcers with osteomyelitis due to medical neglect. The manner of death was ruled a homicide…
