Longhorn Steakhouse locations in Manassas, Virginia, and Fort Wayne, Indiana, also failed inspections this year. For steak lovers, it might be wise to explore other options for now.

2. Chipotle

Chipotle, known for its customizable Mexican bowls and burritos, has a history of health violations that extend beyond just messy tables. The chain faced a significant criminal lawsuit between 2015 and 2018 stemming from widespread foodborne illness outbreaks.

More recently, a Chipotle in Pensacola, Florida, was temporarily closed after inspectors discovered rodent droppings and other clear signs of rodent activity, alongside violations for improper dish sanitization and temperature control for chilled items. Another Florida location in Ocala was shut down due to live cockroach activity, suggesting a need for increased vigilance in the Sunshine State.

3. McDonald’s

The cleanliness standards at McDonald’s can vary widely by location, but the chain recently grappled with a multi-state E. coli outbreak. The outbreak, traced to one of its suppliers, Taylor Farms, led to a reported 104 cases, 34 hospitalizations, and one death, according to the Centers for Disease Control and Prevention.

McDonald’s has since ceased sourcing from Taylor Farms’ Colorado Springs facility. FDA inspectors also reported extensive biofilm buildup on equipment, which contributed to cross-contamination issues.

In Columbia, Missouri, more than half of the five Taco Bell locations inspected this fall were cited for violations. Beyond typical health code concerns, The Cheesecake Factory is facing disturbing criminal charges in Pennsylvania this year, including accusations of forced labor and human trafficking. The lawsuit alleges the chain recruited undocumented workers and compelled them to work unpaid, often without bathroom or meal breaks, and sometimes while sick or under extreme conditions. It’s hard to imagine how a salad restaurant could severely compromise food safety, but Chopt has faced truly alarming incidents. In 2023, a customer reportedly found a chopped-off finger in their salad.

Shockingly, the chain allegedly failed to file the incident with state health officials as required by law, resulting in a mere $900 fine. Further concerns emerged in 2024 when Chopt was taken to court again, this time concerning an E. coli infection.

8. Wendy’s

Wendy’s recently settled 39 lawsuits across five states, paying over $4.7 million for serving E. coli to customers in 2022. These court cases, which concluded this year, highlighted numerous health code violations at various locations that contributed to the outbreak. It’s certainly a spot worth considering twice before ordering your next burger or chicken sandwich.
