A structural failure has been determined to the cause of a helicopter crash in Huntington Beach on Oct. 11 that injured five people.
The Federal Aviation Agency and the National Transportation Board investigated the incident and determined that two small metal arms that connect the tail rotor blades and provide direction of flight were cracked, causing the tail rotors gear box to come lose and push the aircraft out of control.
