A Lafayette man died and two others were injured Sunday in a industrial accident at a plant in Belle Rose, according to Assumption Parish officials.
The deceased has been identified by the Assumption Parish Coroner as 60-year-old Keith Foreman of Lafayette.
The incident occurred around 3:40 p.m., when workers were performing a pressure test on a brine well that was not in service. According to reports, a fitting failed, causing a high-pressure hose to break loose and strike the crew. Foreman died at the scene.
