Kentucky OSHA fines Shively factory months after fatal explosion

LOUISVILLE, Ky. — Months after a fatal factory explosion in Shively, Kentucky’s Occupational Safety and Health Administration (OSHA) has fined the company.

An explosion at the AAK plant, which processes vegetable oils, killed one employee on Sept. 21, 2025.

Shively Fire officials determined the explosion “occurred from a hydrogen tank conversion process.”…
