PHOENIX (AZFamily) – Records from the state health department show repeat violations and thousands of dollars in fines at a Phoenix assisted living and memory facility. Former caregivers and employees told Arizona’s Family that the care at Brookdale Central Paradise Valley is subpar and puts residents’ safety at risk. Additional evidence from the Department of Health Services shows that the staff may not always be qualified for the job.
