December 16, 2025 – Healthcare Network is pleased to announce it has received a $40,000 grant from the Collier Community Foundation, expanding the organization’s capacity to provide enhanced telehealth services for 500 high-risk pregnant and postpartum patients, as well as uninsured or underinsured individuals living with chronic conditions.
These funds will support the Remote Patient Monitoring (RPM) Program, a form of telehealth that improves quality of care for chronic disease treatment and reduces hospital readmission rates, with the purchase of 250 sets of Bluetooth-enabled RPM devices.
Tools such as blood pressure cuffs, scales, glucometer kits, and secure gateway (monitoring) devices will help capture and transmit real-time patient data directly to Healthcare Network, allowing medical teams to respond swiftly to concerning health trends.
