Richmond Exec Charged in Allegedly Billing Medicaid $49M Via Fake Teams, Hotel Kickbacks & Forged Files

A Richmond, Virginia mental health agency executive was arrested Wednesday after federal prosecutors charged her with submitting more than $49 million in fraudulent Medicaid claims over four years — including billing for her own clinical participation on dates when she was not in the United States.

The arrest of Mikia Antoneice Noble, 37, of North Chesterfield, exposes a widening vulnerability in Virginia’s behavioral health reimbursement system: a billing tier designed to fund intensive crisis care became, prosecutors allege, a vehicle for an eight-figure theft from one of the country’s most means-tested safety-net programs.

The Agency Behind the Scheme

Noble served as the 49% co-owner and Chief Operating Officer of Advancing Communities Everywhere LLC (ACE), a Richmond-area mental health provider that marketed itself as a crisis stabilization resource for individuals experiencing acute behavioral health emergencies. She also held the title of Vice President at Community Culture for Virginia (CC4VA), a nonprofit she and her majority co-owner created alongside ACE.

In practice, prosecutors contend, sessions billed as team-based care were routinely carried out by a single lower-credentialed staff member with no licensed professional present.

The Double-Billing Mechanism

The scheme’s financial engine was Virginia Medicaid’s “Team Treatment” billing rate — a higher reimbursement tier reserved for cases where a licensed mental health professional and at least one other trained provider work simultaneously with a patient in crisis. Prosecutors say ACE routinely billed at that elevated rate even when only a single lower-credentialed worker was on-site, a practice that effectively doubled the agency’s revenue per session without doubling the care delivered…
