Bollywood Pizza received a yellow placard following a routine inspection by the Sacramento Health Department on November 7. The inspection report noted several violations, including food debris on kitchen equipment and unlabeled storage containers.

According to the health inspection report, the restaurant was found to have made several violations. Some of them are:
- The California food safety certificate was not available for review. Food handler cards were also missing and should be made available within 30 days, the report said.
- Two bars of paneer cheese were found at 60–68°F and were moved to the walk-in cooler. Cut tomatoes and three types of chicken toppings were at 44–48°F in the prep cooler. The report noted that all potentially hazardous foods must be kept at 41°F or below.
- Food residue was found on the onion slicer, tin blade cutter, and mushroom cutter.
- Two containers of ingredients in dry storage were found unlabeled but corrected during inspection.
- Dust and grease buildup was noted on hood filters, with leftover food found on the door handle and floor of walk-in coolers. The report required these areas to be cleaned within 48 hours.
- A soiled wiping cloth was used to clean the pizza cutter without a sanitizer solution. The report required sanitizer buckets to be maintained at all times, and this was corrected on-site.
- Toilet paper was unavailable during inspection, but was replaced immediately. Two toilet paper holders were broken.
