- The facility does not have an employee with a Food Safety Manager Certificate.
- The report noted one expired food handler card.
- Ham and chicken nuggets stored in the prep-top cooling unit measured 51-53°F after two hours. The items were immediately relocated. Raw shrimp measured 49°F in the lowboy cooler, while cooked chicken, carnitas, fresca, raw eggs, chili, cooked beef, and chorizo measured at 43-45°F in the walk-in cooler.
- The report noted black debris inside a bottle of tequila in the drink storage unit.
- Light slime covered the ice machine’s interior panel, while the two-door cooler prep top had heavy old food buildup and black stains.
- Around 30 old mouse droppings were inside a non-working cooler storing cups and liquor bottles, according to the report. The droppings appeared dry and light grey.
- Dirt, old food debris, and grime were observed on fan guards in the walk-in cooler, the floor behind the ice machine, and the storage room near the office.
- Inspectors noted deteriorated or torn rubber door gaskets on the two-door prep cooler, walk-in cooler, walk-in freezer, and three-door prep cooler. The ambient temperature of the walk-in cooler was observed at 43°F.
- Heavy ice condensation was observed on the rubber curtain of the walk-in freezer, the report said.
- The report mentioned that the last inspection report was unavailable for review. It said that the facility must keep this report readily available upon request.
- The report noted that a dish machine had been added to the bar without approval, marking a repeat violation.
- The facility’s health permit has expired. The report requested the establishment to provide proof of a valid health permit.
