The following establishments were inspected by the Madison County Health Department the week of September 28.

September 29

Taco Bell- Granite City

  • 2 (C) The food establishment is not under the operational supervision of a Certified Food Protection Manager that is present at appropriate times. Provide an adequate number of staff with approved training. Reference 2-102.12. FACILITY ONLY HAS 1 CERTIFIED FOOD MANAGER WHICH CANNOT BE PRESENT AT ALL TIMES. PROVIDE ENOUGH CERTIFIED FOOD MANAGERS, AT LEAST ONE CERTIFIED FOOD MANAGER TO BE PRESENT ON EVERY SHIFT. RECHECK.
  • 39 (C) Food shall be protected by contamination by storing it: 1) in a clean, dry location; 2) where it is not exposed to splash, dust or other contamination; 3) at least 6 inches above the floor. Reference 3-305.11. OBSERVED BOXES CONTAINING FOOD STORED ON FLOOR IN WLK IN COOLER. PIC MOVED ALL BOXES TO SHELVING. CORRECTED.

Kristy Food Mart, Inc- Madison

  • As of January 1, 2019, category 1 and 2 establishments shall be under the operational controls of a person in charge. A person in charge shall be on the premises at all times food is handled, prepared, or served. All employees who are designated a person in charge shall have a current ANSI accredited Certified Food Protection Manager. CFPM STILL NOT PRESENT. DISCUSSED ADJUSTING HOURS OF STAFF TO HAVE A CFPM ON-SITE.
  • 16 (Pf) Food contact surfaces and utensils shall be clean to sight and touch. Wash, rinse, sanitize routinely. Reference 4-601.11. ESTABLISHMENT NOT ALLOWED TO USE ICE FROM MACHINE. ENSURE CLEANING PER MANUFACTOR DIRECTIONS. VISIBLE ACCESS NEEDED AT FOLLOW-UP. HOSES SHALL BE CLEANED/REPLACED. BAGGED ICE FROM AN APPROVED SOURCE IS TO BE USED INSTEAD OF ICE FROM THE MACHINE. KNIVES AND PIZZA CUTTER OBSERVED DIRTY. ALL ITEMS PLACED IN 3 COMPARTMENT SINK.
  • 28 (P) Toxic materials must be used so that they cannot contaminate food, equipment, utensils, linens or single-service articles. Reference 7-202.12, 7-201.11, & 7-301.11. RAID SPRAY BOTTLE STORED NEXT TO WARMER. BOTTLE REMOVED AND PLACED IN STORAGE AREA. CORRECTED.
  • 35 (Pf) Time and temperature control for safety (TCS) food must be thawed: (A) Under refrigeration that maintains the food temperature at 41F or less; or (B) Completely submerged under running water at a water temperature of 70F or below, with sufficient water velocity to agitate and float off loose particles in an overflow, and for a period of time that does not allow thawed portions of ready-to-eat food to rise above 41F; or (C) As part of a cooking process; or (D) Using any procedure if a portion of frozen ready-to-eat food is thawed and prepared for immediate service in response to an individual consumer’s order.   • 45 (C) Single-Use Articles are stored incorrectly. Single-Use Articles shall be stored: 1) in a clean and dry location; 2) away or protected from contamination; 3) at least six inches above the floor; 4) covered or inverted. Reference 4-903.11(A) and (C). BOXES OF INSULATED CUPS STORED ON THE GROUND IN BACK AREA. PIC PICKED UP BOXES AND PLACED THEM IN SHELF. CORRECTED.
  • 45 (C) Single-Use Articles are stored incorrectly. Single-Use Articles shall be stored: 1) in a clean and dry location; 2) away or protected from contamination; 3) at least six inches above the floor; 4) covered or inverted. Reference 4-903.11(A) and (C). BOXES OF INSULATED CUPS STORED ON THE GROUND IN BACK AREA. PIC PICKED UP BOXES AND PLACED THEM IN SHELF. CORRECTED.
  • 51 ( C) Plumbing needs repair. Repair according to Illinois and local plumbing code. Reference 5-205.15. MOP SINK LEAKING. REPAIR OR REPLACE.
  • 55 (C) The premises shall be free of items that are unnecessary to the operation or maintenance of the establishment. This includes equipment that is nonfunctional or no longer used. Reference 6-501.114. WARMER AND COOLER IN KITCHEN NOT WORKING NEED TO BE REMOVED.

Granite City Express- Granite City

  • 10 (Pf) Handwashing sink shall be provided with liquid soap. Reference 6-301.11; Handwashing sink shall be provided with paper towels, continuous towel system supplying user with clean towel, heated-air hand drying device or air-knife system Reference 6-301.12. NO SOAP OR PAPER TOWELS IN MAIN HAND SINK. BOTH PROVIDED DURING INSPECTION. CORRECTED.
  • 28 (Pf) Containers of toxic materials must be clearly and individually identified with the common name of the material. Person in charge labeled the container. Reference 7-102.11. SPRAY BOTTLE WI5 FABULOSO NOT LABELED. BOTTLE LABELED DURING INSPECTION. CORRECTED.

7-Eleven- Granite City

  • 39 (C) Food shall be protected from contamination. Reference 3-307.11. FOODS AT OPEN AIR SELF-SERVE CONDIMENT STATION WITHOUT LIDS TO PROTECT FROM POSSIBLE CONTAMINATION. ORDER PLACED FOR NEW CONTAINERS WITH LIDS. DISCUSSED USING FOIL/PLASTIC WRAP UNTIL NEW CONTAINERS COME IN.
  • 47 (C) Gaskets on the cooler door are cracked or missing. Replace the damaged or missing gaskets. Reference 4-501.11. GASKETS ON COOLERS/FREEZER THROUGHOUT FACILITY NEED REPAIRED/REPLACED.
