Woman federally charged after allegedly billing Utah Medicaid $4 Million in false charges

SALT LAKE CITY (ABC4) — A woman has been federally indicted after allegedly submitting over $4 Million in false claims to Utah Medicaid.

Jazzamin Tashay Clark, 43, has been charged with eight counts of wire fraud, eight counts of Health Care Fraud and five counts of money laundering.

She is reportedly the registered owner of Creative Minds LLC and Luv Well Care LLC, businesses that were advertised as providing substance use disorder treatment and approved as Utah Medicaid providers…
