The incident occurred on May 13, 2026, during an after-school activity held in one of McKinley Elementary’s multipurpose rooms. According to the boy’s family, he was killed when a table fell on him, causing fatal injuries. Burlingame School District Superintendent Marla Silversmith said in a letter to families that the incident is being thoroughly reviewed through proper channels to determine the cause. She noted that all multipurpose rooms in the district were temporarily closed and that procedures related to the use of district facilities are being evaluated.

Millbrae Elementary School District Superintendent Lisa Hickey confirmed that Gavin was a student in another district.
