The Brain Injury Association of South Carolina (BIASC) is offering a a Brain Injury Survivor Wallet Card. This wallet card helps survivors communicate with others who may not recognize the signs and symptoms of brain injury. The card is a printable, fillable form designed to be trimmed to fit in your wallet. The accompanying letter explains the importance of the physician’s signature to support the card’s use.
The card is small enough to be carried in a wallet, and will assist survivors when they come in contact with others who are not aware of the signs or symptoms of brain injury. Often, Traumatic brain injury survivors are not able to express themselves in make others aware of their condition. They are often presumed to be intoxicated, combative, or unwilling to cooperate in certain circumstances.
Those with brain injuries are instructed to:
- Print the wallet card and accompanying letter here.
- Fill in their information on the card.
- Present the letter and card to their physician.
- Ask for the physician’s signature on the card.
- Trim out the card and keep it in their wallet.
