Report details 2019 refinery incident that could have prevented deadly 2022 blast

OREGON, Ohio (WTVG) – It’s a question with life-and-death implications. Could changes made at a Northwest Ohio refinery back in 2019 have prevented a deadly accident at the Oregon plant just three years later?

That’s one of the findings in a federal report and animation released this week by the US Chemical Safety and Hazard Investigation Board.

The report also illustrated a very similar problem in 2019 when some called for a more permanent fix which appears not to have happened…
