SAN ANTONIO – Federal inspection records obtained by FOX San Antonio detail a series of serious deficiencies at Laurel Ridge Treatment Center, including findings that placed patients in what regulators described as Immediate Jeopardy, the most serious level of deficiency issued when patient health and safety may be at risk.
According to federal records, inspectors found the facility failed to adequately protect patients from hazards inside the hospital.
