An Iowa licensed practical nurse has been disciplined by the Iowa Board of Nursing following the death of a resident at the Klein Center Nursing Facility in West Burlington.
The Board issued a citation and formal warning to Samantha Smith after determining she failed to properly assess and respond to a resident who appeared to be choking in December 2025. According to the Board’s order, Smith placed the resident in bed with a CPAP machine without completing additional vital assessments.
