Charlotte Man Facing Felony Fraud Charge Over Fake Doctor’s Note He Gave To His Boss

A Charlotte man faces a felony insurance fraud charge after investigators say he used a fake doctor’s note to seek reimbursement.

North Carolina Insurance Commissioner Mike Causey announced the charge Friday against 42-year-old Henry Lee Clyburn Jr. Investigators say Clyburn submitted the note to Progressive Insurance Co. after an automobile accident. The document allegedly claimed a doctor advised him to miss work because of accident-related injuries.

Investigators say Clyburn never visited the doctor named on the note, alleging that he submitted the document to obtain reimbursement for missed workdays…
