Duke employee health insurance premiums to rise by 10% to 50% under new salary-based system

Health insurance premiums for Duke employees are going up next year — by a lot.

Duke Human Resources announced Tuesday morning it would introduce a new five-tier, salary-based health insurance premium structure for full-time employees in 2027, replacing its current flat-rate system. The change, which comes in response to what Duke HR calls “non-sustainable” rising healthcare costs, would cost some employees 50% more in monthly premiums.

Under the new system, employees will pay different portions of their total health insurance premiums depending on their salaries. Employees in lower salary tiers will pay a smaller share of their premiums with Duke covering the rest, while those in higher tiers will pay a larger share…
