Five Trinity Health hospitals across Southeast Michigan have banded together for their first shared Community Health Needs Assessment survey. Trinity Health Ann Arbor, Chelsea Hospital, Trinity Health Livingston, Trinity Health Livonia, and Trinity Health Oakland will mine the data to pinpoint and tackle health gaps in each area they serve.
The survey runs until July 31, 2026. Anyone living in Livingston, Oakland, Washtenaw or Wayne counties can complete it. No names required.
