Woman who ran unlicensed care facility in Greensboro where man disappeared pleads guilty

GREENSBORO, N.C. (WGHP) — A woman charged with operating an elder care facility without a license, where a man disappeared and is still missing, pleaded guilty on Tuesday.

Hazel Mae Forman, 51, pleaded guilty to misdemeanor running an adult home care without a license and was fined $50, according to court documents.

Disappearance of Larry Burton

Larry Burton, 74, was last seen at the HMF Family Home Care facility in Greensboro in July 2025 and reported missing shortly after arriving.
