OSHA fines H-E-B over workplace death

Federal safety regulators have fined H-E-B nearly $18,000 for violations related to an October incident at a company warehouse that left a worker dead and questions about what happened.

The Bexar County Medical Examiner’s Office said Teresa Dominguez, 27, died of blunt force trauma to the left lower extremity in what it deemed an accidental death that occurred at the grocer’s site near Interstate 35 and Rittiman Road.

Initial police reports said Dominguez was found unconscious inside a warehouse freezer and had possibly been injured by a forklift, but H-E-B later said that was inaccurate…
