Van Duyn nursing home cited for immediate jeopardy violations despite reforms

New state and federal inspection findings are raising fresh concerns about care at Van Duyn Center for Rehabilitation & Nursing, where regulators have cited the Syracuse facility for multiple “immediate jeopardy” violations — the most serious level of nursing home infraction.

One investigation by the U.S. Department of Health & Human Services’ Centers for Medicare and Medicaid Services details a February incident involving a brain-injured resident who told staff they did not want to be touched during a diaper change. Inspectors said a male certified nursing assistant aggressively pushed the resident’s face after the resident spat at the worker, leaving the resident with cuts and scratches to the face and neck.

The state also found staff failed to follow the resident’s care plan, which prohibited male caregivers from treating that resident. Van Duyn said the nursing assistant was fired…
