BATON ROUGE, La. (WAFB) – A Baton Rouge nursing home off of Sherwood Forest near I-12 has been designated by federal regulators as one of the most problematic in the country, carrying a label that requires immediate improvements.
White Oak Post Acute Care is listed by the Centers for Medicare and Medicaid Services as a Special Focus Facility, a designation reserved for nursing homes with serious, persistent problems. It is the only facility in Louisiana currently carrying that label.
